I couldn't find any games after 8...h6 h4. I looked this over a little both with my own eyes and through computer (Stockfish) evaluations.  White move seems to be sound. It looks scary, but perhaps it's so dangerous. It seems that black usually cannot capture the knight, but the knight is really not threatening anything immediately. Both players should just go about their business.

Here's a quick sample that played out better for black:
1. d4 d5 2. c4 e6 3. Nc3 Nf6 4. Nf3 Nbd7!? 5. Bf4 c6 6. e3 Be7 7. Qc2 O-O 8. Ng5 h6 9. h4 b6 10. cxd5 cxd5 11. Bd3 Bb7 12. f3 Rc8 13. Kf2 a6 14. g4 b5 15. a3 Nb6 16. Nh7 Nxh7 17. Bxh7+ Kh8 18. g5 Nc4 19. Rag1 h5 20. Bd3 e5 21. dxe5
d4 22. exd4 Qxd4+ 23. Kg3 Nxb2!

Not best play, I'm sure. In general, I think black gets equal play and an interesting game.

"Theory" has given attention to 5...dxc 6e3 and now 6...Nb6, 6...Nd5, or 6...b5. I think each move is reasonable and worth a try. I remember that an older book (Lalic?) showed enthusiasm for the knight moves. 6...b5 is newer and both Chess Publishing and Nigel Davies are enthusiastic.

I don't think that any of these moves are strong enough to discourage white from playing 5Bf4 from your move order.

Another approach has received less attention is 5...Bb4. If white's bishop were on g5, this would be a Ragozin or Manhattan system. Posting the bishop g5 seems stronger to me.  So I think 5...Bb4 could be very promising. I'm not aware of any theory on this.

The most thorough coverage I've seen on 5Bf4 systems was in a book by Colin Crouch. It came out early and was very complete and good. It's not available to me right now though.

My database contains the game Rasmussen Karsten (DEN)-Wegerer Fred(AUT) It (open) Pula (Croatia) Round 5 20070626. Black lost the game, but had his chances:

1. d4 d5 2. c4 e6 3. Nc3 Nf6 4. Nf3 Nbd7 5. Bf4 Bb4!? 6. cxd5!? exd5 7. e3 Ne4 8. Qc2 g5!? 9. Bg3 h5 10. h4 Nxg3 11. fxg3 11. fxg3 g4 (...gxh) 12. Ng5 Qe7 13. O-O-O (Bd3) Bxc3 14. bxc3 Nb6 15. Bd3 Bd7 16. Rhf1? (Rdf1) f5? (Ba5!) 17. Bxf5 O-O-O 18. e4 Nc4?! (Bxf5 or dxe are better though it's no obvious) 19. Bxd7+ Rxd7 20. Qa4 Nb6, and now white should simply play 21Qxa7 with the better game.
