He recommends to play 1.d4 d5 2.c4 c6 3.Nf3 Nf6 4.e3 e6. But he also recommends 1.d4 d5 2.c4 e6 3.Nf3 Nf6 4.g3 and I could not find any thought given to 4...c6
What to do?

This is covered in the pgn update on the Quality Chess site. Otherwise it's going to be a transposition to other lines after 5.Bg2.

I find a gap in Avruch's otherwise fine coverage. After 1 d4, d5; 2 c4, e6; 3 Nc3, c6; 4 Nf3, Nf6; We are in a Semi-Slav but there is no way to get into Avruch's recommended line on p. 300 because of the early Nc3 by White: 1 d4, d5; 2 c4, c6; 3 Nf3, Nf6; 4 e3, e6; when Avruch's recommended line is 5 b3 instead of the normal Meran line 5 e3.
Thus, in the first move order, W has two choices: 5 Bg5 leading to the sharp Botvinnik or Moscow System lines or 5 e3 Meran Defense.
Therefore, I would appreciate any recommended books or resources (for White especially) on the Meran Defense.

? In that case after 3...c6, White can go for the Catalan setup with 4. g3 as covered in the pgn update. So I don't think White must go for the main line Semi-Slav.

4. g3 versus the Noteboom is well met by dxc4. Black isn't obliged to enter Catalan territory.
White's better off with 4. Qc2 (Khalifman), 4. Nc3 (Schandorff), or 4. e3 (Avrukh) in that move order.
