The Catalan is covered at the end of the QGD book (E00-E09 is the Catalan). It's just covered in a QGD move order instead of a Nimzo move order. I haven't looked much at the Nimzo/Benoni section as I don't end up in that territory (I play 1.Nf3, go Anti-Nimzo against that set up, and transpose to QGD, KID, Old Indian, Dutch, etc. and against 1.d4 I play 1...e6 and if 2.c4 I play 2...d5, so no Nimzo for me), but 1.d4 d5 2.c4 e6 3.g3 Bb4+ won't always, but will often transpose to a Bogo-Indian assuming Black eventually plays ...Nf6, so you might look there. To answer your question as to why does everyone cover it with the moves you mentioned...it can also come from 3.Nf3 and 4.g3, but that just transposes to the 5.Nf3 lines anyway. As for other moves on move 3, they just transpose to other openings, and therefore belong there. For example, if 1.d4 Nf6 2.c4 e6 3.g3 c5, you end up most of the time transposing to a Fienchetto Variation of the Benoni, which would be in "Nimzo & Benoni", while 3...Bb4+ would probably be in the lines of the "Bogo-Indian", and if not exactly that, most of the time it will transpose to a line of the Bogo. However, I don't seem to know of any opening that would transpose to the Orthodox QGD that would cause me to figure it to be in another section other than 1.d4 d5, but alas, there is nada in the D60's. I guess they assume there is no such thing any more as 1.d4 d5 2.c4 e6 3.Nc3 Nf6 4.Bg5 Be7 5.e3 0-0 6.Nf3 Nbd7. I seem to recall seeing this line quite a bit, and in the past I have played 7.Qc2, but have been looking to perhaps learn the 7.Rc1 lines. It seems, however, that 6...Nbd7 must be refuted or something since it seems to get ignored here....apparently the only good move now is 6...h6 and 7...b6. Very Depressing. Not depressing enough to discontinue subscribing, as I get lots of info from the French (Which I play religiously as Black), Dragons (1.Nf3 often transposes to Maroczy-Bind positions of the accelerated), d-pawn specials, King's Indian, Daring Defenses, and Flank Openings, but the d4 d5 section gets limited attention as it lacks the most important of all d4 d5 lines (luckily enough, at least the Slav and Semi-Slav are covered). I guess 6 out of 7 isn't bad (can't validly comment on the other 5 sections as they really don't apply to my reportoire).
