In the line given by da Costa Junior, 19...Rb4! 20 b3 a5! seems a sizeable improvement

Well I had thought that (10.exd5) was strongest as well, but you might want to take a look at Da Costa Junior's post under the heading "new main line in 9.000 d5".

This is not where white is struggling for advantage. I believe black is ok after sacking the queen for rook and minor. however, d5 exd5 nxd5 nxc6 bxc6 bd4 e5 bc5 be6 ne4  is why most gms don't play this anymore. There are other ways to play, such as nxc3 qxc3 bh6+ and some other tries, but I think they are not enough to equalize.

What is White's way to advantage after:

1.e4 c5 2.Nf3 d6 3.d4 cxd4 4.Nxd4 Nf6 5.Nc3 g6 6.Be3 Bg7 7.f3 Nc6 8.Qd2 0-0 9.0-0-0 d5 10.Kb1 Nxd4 11.e5 Nf5 (one of the main lines you mentioned) 12.exf6 Bxf6 13.Nxd5 Qxd5 the queen sac line?

The main line 1.e4 c5 2.Nf3 d6 3.d4 cxd4 4.Nxd4 Nf6 5.Nc3 g6 6.Be3 Bg7 7.f3 0-0 8.Qd2 Nc6 9.0-0-0 d5 10.Kb1 Nxd4 ('book' move) and after 11.e5 black has two options 11...Nxf3 and 11...Nf5 but not enough to equalize the position (Small ECO, 2nd edition, 2003).

I think 10...Rb8!? is an interesting idea and I played one game in the line 11.g4 e5 but I think white has two good options: 11. h4!? and 11.Ndb5!? and it's not easy to equalize the position.

10.Kb1 may become the main line for white to play against 9.0-0-0 d5

Does anyone know what the most effective antidote for Black versus 1.e4 c5 2.Nf3 d6 3.d4 cxd4 4.Nxd4 Nf6 5.Nc3 g6 6.Be3 Bg7 7.f3 Nc6 8.Qd2 0-0 9.0-0-0 d5 10.Kb1 is?
