Partly, Markovich, as it can also arise via a Nimzo and other openings. Hence my interest in it.

I can confirm that there is such a book, written perhaps a decade ago by a US master.  But I don't remember the details, and I only glanced at the book some years ago.

(update:  yes, jeupham has the one i remember)
