As far as I know there´s no adaequate coverage from black´s side.

Modern Defence by McDonald/Speelman (Everyman 2000) has two chapters (35 pages) about 1e4 g6 2d4 Bg7 3.Nc3 c6 and 4...d5 .

The best coverage is undoubtly Khalifman with his OfWat Anand 3 (2004), via different move orders he analyse both 4 Nf3 Bg7 5 h3 and 4 e5 Bg7 5 f4 (and also 4 e5 --- [d6-d5] 5 f4). Of course everything is from white side and over all at least += ...

Obviously ...g6 Caro-Kann has disappeared from higher levels but there are still many practioners ~2300-2500 in international tournament scene.

I'm guessing that they employ it to surprise white more than anything.

Maybe you should ask what books on the Robatsch/Modern etc. treat this defence. Nunn/McNab's The Ultimate Pirc has a chapter devoted to 1.e4 g6 2.d4 Bg7 3.Nc3 c6 and 4...d5.

The book had the effect good books usually have: it made the stupids more stupid, the intelligent more intelligent and the other thousands of readers remained unchanged.

Which books on the Caro-Kann have good coverage of lines with an early ...g6, for example 1.e4 c6 2.d4 d5 3.Nc3 (or 3.Nd2) g6 ?
