2.Bc4 Nf6 3.d3 e6 4.Nc3 d5 and the bishop does not make sense to me. It just looks like a good French where White loses more time than Black with a misplaced bishop.

Playing black, I followed some of the advice given, although opponent didn't follow exact white moves described: I now have my Queen on b6 attacked by Be3. I have pawns on c6 and d6 with Knight on d7. Should I block with c5 or Nc5? or move Queen?

After 1.e4 d6 2.Bc4 Nf6 3.d3 c6 4.Nc3 - I think playing 4...d5 looks good for black.

Help against what?
Castling Queenside after 1...d6 ? That is a bit rare. If you have faith in the soundness of your setup you shouldn't be so afraid that you need this option.
Sample line:

1.e4 d6 2.Bc4 Nf6 3.d3 c6 4.Nc3 b5 5.Bb3 a5 6.a4 b4 7.Nce2 Nbd7 8.f4 Qb6 9.Nf3 e6.
White has problems with castling and I don't see a trace of an attack by White.

I thought of that, but I couldn't see how it would help. Doesn't it just rule out the possibility of wrong-footing White's aggressive intentions against the kingside by castling long?

Another idea is to harrass the Bishop with ...c6 and ...b5.

For a while now I have been playing the Hanham Philidor, via the 1...d6 move order. After 2.d4 Nf6 3.Nc3 e5 I nearly always get a perfectly decent position, and quite often win. But a large proportion of my opponents (at around 1600 level) play 2.Bc4, followed by d3, Nc3 and f4. If I play ...e5 against this set-up I tend to find myself on the wrong end of a KGD-style crush. Is it better to change tack and play ...e6, aiming for a quick ...d5?
