Still probably the best for understanding the Leningrad is the Kindermann book. The recent Malaniuk and Someone book is good too, more up-to-date but with less explanation.
And flip, yeah a4 is useful. h3 is rarely out of place and often useful (the e6 square, no knight, plus k-side g5 push), perhaps Kh1 (no d5 checks), but that last might be a bit...overt.

Not really. But I think that's part of its appeal. I think the best book I had when I was working on this line was Simon Williams's Play the Classical Dutch. It provided good instruction on the Dutch, and there was some fun involved in reversing the position and working out how to take advantage of the extra move (I typically opted for 7.a4).  Play through some database games and develop a feel for the Reversed Leningrad.

I don't know about a whole book, but James Vigus devotes a chapter to it ('Beware the Polar Bear!') in 'Dangerous Weapons: Flank Openings', as does Timothy Taylor ('The Leningrad Bird') in his book, 'Bird's Opening'.
