I know, it's a small side step and 3.Nc3 de4 4.Ne4 Bf5 is in good shape, but why isn't Karpov's 4. ...Nd7 no topic anymore?

Fernandez' new book covers it.

Why are there 2 topics on this book and is this one not in the Caro-Kann section?

I find the Smyslow/Karpow 4...Cd7 to be similar to the French Rubinstein. Solid line that is slower than 4...Af5 like how Rubinstein is to 3. Cc3 rather than Winawer/Classical or 3. Cd2 to 3...c5/3...Cf6/3...other. But having to choose between 4...Cd7 and 4...Af5, where the latter is both more active and known to be in excellent shape, most go for 4...Af5.

Even in that case, to choose both 4...Af5 and 4...Cd7 would probably be too much for one book. His 1...e5 books where he covered the Marshall and Brejer were two volumes instead of one.

The older, the better - over 2200 and still rising.

I'm pretty sick of seeing 7...Nd7 recommended (though I don't play the Caro-Kann).  I wonder if he offers anything substantially different from what's been covered in the past?

My feeling is that White has nothing in either 7...Cd7 nor 7...e6. But 7...e6 is newer and I think it is just a better choice for Black. I would have liked to see 7...e6 covered in addition to 7...Cd7.

Any White attempt for advantage seems now to be all placed in the Adwance. The last time I saw 3...Af5 for Black was in Schandorff (2010).

The line with 5...c5 is completely different from 3...c5, in fact I think now it is some sort of main line, or at least one thereof. I cannot tell which line is it exactly, but that 5...c5 looks similar to the line on Vidit's DVD.

You can often meet .. c5 with c4. That's one of the earlier side variations to be identified. Back in 1991 I met 5. .. c5 with 6. 0-0 and then 6. .. Nd7 with 7. c4

I see he still recommends an early c5, if not at move 3. As white I've always found that a bit easier to play against than some other lines, where it can be easy to become confused by the lack of threats and do something weakening. Arguably that's what at least some players of Black are attempting to provoke by playing the Caro, that White will self destruct.

Personally I like the line where Black does this setup: ...Ce7/...Cd7/...h6/...g5/...Ag7/...Cg6. White has to try some strange plan with Cb3 and push a4/a5 whilst keeping open option for f4. That line can be sharp and positional, all at same time.

3...Af5 against the Advance though. The latest Caro-Kann books all seemed to cover 3...c5 for some reason.

Bologan Caro-Kann book

https://www.newinchess.com/bologan-s-caro-kann

I contacted New In Chess, and they said in three weeks the book should be out. There is now excerpt, but I think it looks like 7...Cd7 instead of 7...e6 in the main line. I am still wondering why books do not want to cover the hot 7...e6.

3...Af5 against the Advance though. The latest Caro-Kann books all seemed to cover 3...c5 for some reason.
