Maybe this reveals my preferences, as some systems I suck at and some I do well in.  Anyway, here are two phases  of my d4 repertoire and the ordering that you ask for in each phase.

Phase 1-

1.QGD Exchange-theoretical darling, overwhelming choice of elite players vs. pure QGD move order.  I suck at it though; I would groan every time I had to face it.

2.Semi-Slav Botvinnik system-I really flopped in the complications for some reason on the White side.  I do much better here with black. Weird.  Something about seeing  that triangle of pawns in the semi-slav produces a reaction of awe and respect (maybe fear!?).

3.QGA Nf3 Classical Systems- not good at the IQP here.  I like to be black instead.

4. Nimzo e3- I used to start feeling in my games that black was actually better in these positions.  Not a pleasure for me.

5. Slav- I really enjoy playing vs. the Slav actually, with both Ne5 and e3.

I made some adjustments, here's phase 2:

1. QGD Catalan- I do WAY better with the Catalan, but it is still a tough opening for me to face.

2. Semi-Slav Anti-Meran Qc2:  Thank you Sakaev, I've found my calling.  Still a difficult and complicated setup to face, though.

3. QGA- e4: I like playing vs. the QGA in this line, even the endings.

4. Slav- mostly gone from my Nc3,e3 move order. Sometimes I use the reg. move order though and play the Dutch system.

5. Nimzo Qc2- LOVE this opening from both sides of the board.   

I guess a short summary of my tastes is this: don't like IQP with white, prefer flexible or open pawn structures in the center, with one exception: the advance french center (d4,e5 vs. d5,e6) that sometimes I can get in the Slav/Classical/Dutch variation or the Anti-Meran Qc2  quiet lines.

The main line of the Slav is the only thing that really bothers me.  Whether White plays e3 or Ne5, it seems that his advantage is a little nebulous.  I avoided this for a long time by playing the Exchage Slav, but I doubt that that is the theoretically best answer.

I would like to play the Tolusch-Geller Gambit, but I doubt its soundness.

1. I avoid the Nimzo, I take the Bogo if it comes and I like playing against the QID with g3 and a pawn sacrifice on d5, you get a long lasting initiative with pressions on the center and many black players feel uncomfortable in such situations.
2. QGA: I do not like to play against it but I do not try to avoid it. I try to go the catalan way. It is difficult to get some influence on how black will play, so you have to be patient when trying to find a plan.
3. QGD: I like to play against it and I use the catalan. You can get some complications on the Q-side but you have also (very) good compensations in the center and may have the opportunity of starting a strong K-side attack.
4. Slav: Difficult to play against someone who knows what he is doing. I try to the black white-squares bishop off and to get some play on the c-line with possibilities on the K-side.
5. Semi-slav: I like it. I try to go for Qc2, Nc3 and g4. You can get wild games and, as many black players choose the semi-slav because they want to feel secure, they are not comfortable in such positions. 
  

Yusupov once said that “The problem with the Dutch Defence is that later in many positions the best move would be ...f5-f7” but he is surely wrong.

I am a 50% d4 player, but here it goes from least to most:
-QGA. You usually have a slight advantage, but it is very hard to get anything done with it, especially against tricky players.
-Slav, I get it too often so it gets a bit boring.
-QGD, I am still coming to grips with that opening. It is a bit like the Spanish (strategic battles), but without the dynamism.
-Semi-slav. I love the complications, while for some reason the opponent doesnt
-NI. I play it with both sides, because I just love the positions you get with many different structures and thus plans.

The KID would be just below the semi-slav. I win a lot against it, but that is mainly due to me using the bayonet and just aiming for c5 with whatever moves support it. Black usually doesnt get it and just gets crushed on the queenside while he is still thinking about preparing a kingside attack.
  

If nothing else works, a total pig-headed unwillingness to look facts in the face will see us through.
