That's a good question. In general I would think it should be ideas that had a greater historical impact; be it innovative new concepts (which would became popular for at least a certain period of time, preferably till today), the improvement of already existing opening systems (especially those reviving "forgotten" systems) or novelties refuting previously popular opening systems.

It depends on what you understand as great opening. Is this an opening that gives good results, an opening where both players should start thinking very early in the game (or an opening in constant evolution), an opening that demonstrates that till then accepted principles are not the only ones to be right or is it an opening that might not be sound but which amusement degree is high...
  

Yusupov once said that “The problem with the Dutch Defence is that later in many positions the best move would be ...f5-f7” but he is surely wrong.

Karpov-Kortchnoi and that Ng5 in the Open Spanish, don't know if it's still considered good but it was a big surprise to me when I first saw it...

Nimzowitsch, Reti and Aliojin ideas are among the greatest of all time. Sokolsky was also great.

Actually it was Opocensky's idea. He was too obsessed with the e5 move that he eventually developed 5..a6
so that he can enjoy the dynamism of the Boleslvsky & avoid its pitfalls at the same time.

I guess today's mainlines were all new ideas at one time or another. So probably Najdorf's 5...a6 qualifies as 'The Greatest Idea'. But three other lines that must have taken some courage from the inventor come to mind:
- The Evans Gambit 1.e4 e5 2.Nf3 Nc6 3.Bc4 Bc5 4.b4!?
- The Traxler 1.e4 e5 2.Nf3 Nc6 3.Bc4 Nf6 4.Ng5 Bc5!?
- The Muzio Gambit 1.e4 e5 2.f4 exf4 3.Nf3 g5 4.Bc4 g4 5.O-O
