Forget who it was, but a famous story of a GM who showed up drunk and, as Black, bashed out 1.e4 Nc6 2.Nf3 b6 3.d4 e5.  Upon 4.dxe5 our hero stared at the board, then commented that he'd been playing the Gruenfeld his whole life and never before realized that it loses a pawn!!

I once had an opponent who fell asleep during the game. His excuse was that it had been a long day. As I lived even farther away it had been a long day for me as well, but for some reason that was irrelevant to him.
The man woke up every time I hit the clock after moving, so I complicated and he missed some tactics.
  

The book had the effect good books usually have: it made the stupids more stupid, the intelligent more intelligent and the other thousands of readers remained unchanged.

When I played in the Ohio Championships in '04 (I think), GM Aleks Wojtkiewicz ("Wojo") arrived there a day early and got absolutely hammered before his first round game and lost to some kid (who was still quite strong, now over 2400). Then during round 2 passed out in his game with my friend about 13 moves in and remained that way for the entire first time control. When he woke up and my friend told him that he flagged, he was none too pleased, but just grabbed two glasses of wine from the bar and took them back to his room. So sad... Sad

One of the funiest things I have experienced was when one of my clubmates drew a teamgame against a higher-rated player. His opponent was disillusioned by the draw and as a consequence ordered himself 2 Westmalle tripels (Belgian beer; 9,5% alcohol). During the post-mortem he kept ordering and drinking tripels muttering to himself constantly that he should have been beter and that he should have won. By the time his teammates were ready to leave he hardly knew where he was anymore.

I wasn't a first-hand witness, and it wasn't a GM, but I know someone who fell asleep at the board, quite far into the game. He lost the game on time and didn't hear his opponent say "time."
