Experienced arbiters should have heard of this. Allegedly one or two GMs realised that they could exploit the inexperience of arbiters with digital clocks back in the 1990s by playing an illegal move when short of time.

Yes, that is how it would more or less work. However:

- You now have to move or capture the piece you made the illegal move with
- It's going to take far less than five minutes, arbiters make sure they are right next to the boards with the worst time trouble and adding 2 minutes is easy

And again, a good arbiter will see this happening. That means he doesn't have to spend time confirming that Rh1-a8 was illegal, and if he thinks it was done on purpose, then other rules come into play -- the role of the arbiter includes 12.2.a, "ensure fair play", and penalties he is allowed to use include loss of the game.

For playing a ridiculously illegal move on purpose with two seconds left, I think loss of the game is reasonable.

If you get away with it, you won't be able to do it often, reputations are acquired quickly.

FIDE Rules, Section 7.5.b: "After the action taken under Article 7.5.a, for the first completed illegal move by a player the arbiter shall give two minutes extra time to his opponent"

Following scenario:
- You just made your 40th move in a highly complicated position
- Your opponent has 2 seconds left on his clock to make his move

He now plays an illegal move.

You pause the clock, call an arbiter. The arbiter looks at the position, determines that indeed this move is illegal, sets the position & the clock back to what it was previously (removing potential increment, as well as the 40move bonus) and adds 2 minutes to your time.

Those 2 minutes added to your clock are obviously useless, since you just hit move 40. 
Meanwhile, your opponent now had 5minutes -rather than 2 seconds- to look at the position, think up a good move, and play it.

Is this truly in accordance with the rules? 
Or should an arbiter deduct time from the black player (forcing him to lose)?

I'm not really finding any concrete course of action other than the adding of 2 minutes to the opponent's clock, which in this case (and many others) doesn't accomplish anything - 
but it seems strange to me that making this illegal move would provide black with such a tremendous advantage over playing "fair"/correctly?!
