Sorry, but 16.Qd2 cannot be right since d2 is still occupied by the bishop. I guess you mean Qd1. 

In 2009 I played 16.b4 Nb6 17. Qd1 Bd7 18.Nd4 Rc8 and then the attack 19.Bxh6 came into consideration but did not seem sufficient, so I played 19.Rxc8 Qxc8 and then 20.Bxh6 resulting in a quick draw.

So Bxh6 is not easily overwhelming and I would love to see it work after 16.Qd1. Is there any hard proof for this?

Oddly enough I'm playing a corr game in this line. I chose 15. Rac1, keeping my options open. I'm half a dozen moves further so I can't really say much about it but computers tend to underestimate the h-pawn push. Once you help them on their way they begin to see the light. Anyway it is a tricky line.

In Informator 86 I found the game Belotti vs. Huebner, Schweiz 2002, with comments by Robert Huebner. He recommends 13.Bd2!? 0-0 14.Qc2 g6 15.b4 unclear.

     and White has comp for the pawn. An example:
     17...Bd7 18.Ne5 Bd8 (18.Be8 Rac1) 19.Nxd7 
     19...Qxd7 and White has two bishops to add to
     his comp, 20.Rfe1 Qf7
     (20...Kh8! seems best)
    21.Re3!
     (the idea is 21...Ng4 22.Bxh7!)
    21...Bb6
     (else Rh3!)
     22.Bxb6 axb6 23.Qe2 Rfe8 24.a4! Rad8 25.Bb5 
     Re7 26.Re1 Rd6 27.Qd2 with Qd4-f4-b4 and total
     domination. A plan is g3-Kg2 and finally Bd3+f5

But there is no clear solution after 20...Kh8! which makes me believe that tere should be a better move than 20.Rfe1. Any ideas?

I don't understand the need for a quick ...O-O from Black. After 14.h4 Nc5 15.Bc2 h5!? or maybe better 15...Bd7 with the idea Bb5 give plenty of play for Black. I really don't believe that 13.Qg4 is the way for White. Black can also try 13...Bf8!? or 13...Kf8!? without clear advantage for White in either case.

I play this line as Black but I can try to propose something for White...

12.Qa4 Be7 13.Qg4 g6 14.h4! oo (14..Nc5 15.Bc2 oo 16.h5 Kh8 17.Bg5 Bxg5 18.Qxg5 Qd8 19.Qh6 Rg8 20.Ng5 Rg7 21.hxg6 fxg6 22.Bxg6 Qe7 23.Nxh7 Rxg6 24.Qxg6 Qxf7 25.Qf6+ Qg7 26.Qd8+ Qg8 27.Qe7 b6 and now I believe the rook lift on the 3rank is decisive) 15.h5 Qb5 16.Qg3 Nc5 17.hxg6 fxg6 18.Bxg6  +/-

...which is an attack with help of the h pawn

...but Black has 14...Qb4 and ending is equal after queen exchange

After the moves 1.e4 e6 2.d4 d5 3.Nd2 Nf6 4.e5 Nd7 5.Bd3 c5 6.c3 Nc6 7.Ngf3 Qb6 8.O-O cxd4 9.cxd4 Nxd4 10.Nxd4 Qxd4 11.Nf3 Qb6 12.Qa4 i cannot find an advantage for White after the simple 12...Be7! Moskalenko analyses 13.Qg4 g6! where Mc Donald claims unclear play. Mc Donald gives instead 13.Be3 but after 13...Qxb2! White has comp for sure but for 2 pawns? Probably not. Better moves are 13.a3 or 13.Bd2 with the idea 13...Qxb2 14.a3 and the Queens is lost. I have tried to analyse 13.Bd2 O-O 14.Qc2 f5 (or 14...h6) 15.Rac1 (if Rfc1 Bc5 comes with a threat at f2) and now 15...Nb8 16.Qxc8! is great for White but Black can play first 15...Qd8! and now how should White continue? Black will play ...Nb8 with the idea ...Nc6 and if Nb8 Nd4 Bd7 first. 

Any ideas for White? Or should we consider something else. for example 12.Qc2?
