This is covered under the move order 6...c5 6.Ne2 b6. Anyway, it is rather senseless. It was designed to avoid the sac on c5.

It is not dismissed. It is in the 6...c5 part, there is also an annotated game.

Regarding ...c6 and ...a6 in the Sämisch, I would have used the name of a recently deceased American grandmaster, rather than calling it "Extended Black Fianchetto" (when in any case Black doesn't always play ...b5 and ...Bb7).

I have just posted the Contents of the ready-to-print book by Svetushkin at http://chess-stars.com/resources/Contents_Ultimate.pdf. Half of it is on the Sämish though - as it presents a White repertoire with 3.f3.
