A)
1.e4 e5 2.f4 exf 3.Nf3 g5 4.Nc3 Nc6 5.h4 g4 6.Ng1 is probably inferior to 6.Ng5. I analyzed this ten years ago. The only mention of it I have seen is in the book on the Vienna by Jakobetz and Somlai and a short note in the book on 3.Nc3 in the KG by Bangiev and Hergert (1993). Both these books are in German language.
6...d6 7.d4 Bh6 transposes to a known line in the Fischer defence (3...d6 4.d4 g5 5.h4 g4 6.Ng1 Bh7 7.Nc3 Nc6)
Jakobetz/Somlai give a game with 6...h5 7.d4 Bh6 which seems bad for Black.
Bangiev/Hergert recommends 6...f3, which I like. On 7.d4 they give 7...Be7, but 7...f2+ 8.Kxf2 Qf6+ also looks good. After 7.gxf3 Black two good answers in 7...Be7 and 7...Bc5 (w.idea Bxg1 and Qxh4+) 

B)

Fedorov played 6.Nxg4 once, but probably as a surprise weapon. Anyway Black can reach known waters with 6...d6. 7.Nc3 is Bronstein's lazy knight. White has a monster score in this variation. I analyzed 9...Nxh1 back in 2002, while playing White in a correspondence game in the Champions League. I came to the conclusion that White is busted after 10.Bxf4+ Qxh4+ 11.Kd2 0-0. Now there is no check on h6 and if White takes the rook on a8, Black will bring his pieces out for an attack. However, I was going to play 10.Qe2+ Kf8 11.Bxf4 Qxh4+ 12.Kd2 and if 12...Ng3 then 13.Qf3! Yes White loses a tempo, but Black has forfeited the right to castle and his pieces are all at home.
Sample variations:
1) 13...h5 14.Bxg3 Qxg4 15.Qxg4 hxg4 16.Nxc7 followed by Nxa8, Bd6+ and Re1.
2) 13...Nh5? 14.Qa3+! d6 15.Bxd6+
3) 13...Nf5 14.g3 Qd8 15.Nxc7

C) White is still struggling to prove any advantage or an even game that is not dead drawn against 5...d6 6.Nxg4 Nf6 in the Kieseritzky.

D) Yes the double Muzio is busted by Qf5.

E) The Lean attack is also dubious but 7.d3 may be playable if you like to play a queenless middlegame position with two pawns and some pressure for a piece.

Would you consider a four year record of 12½/16 (+11 =3 -2 the two losses from two winning positions) against an average of over 2150 ELO a good result? And yes it was fun too!
I do agree that people who play the Colle or London systems should be condemned to play in closed tournaments against their own kind for eternity.

I wonder why serious GMs who write serious books/videos to make serious money, recommend 2...Bc5, and they all (Marin, Davies, IM Vigorito etc) fail to give White's best moves?
