For White, Black, or both? There's not been a decent book on these lines for Black to my knowledge (and I've looked a lot!). For White, look to Khalifmann's Anand series Book 4 (although I found a equalising line for Black in one variation and 4. f4 Qa5 5. e5 vs the Czech variation might be more dangerous for Black than 5. Bd3) and/or Carpathian Warrior (if your eyes can stand the cover  Wink)

  

Those who want to go by my perverse footsteps play such pawn structure with fuzzy atypical still strategic orientations

as it happens McDonald gives this in Starting Out: 1 e4
though obviously, as a Starting Out book, it's not very deep

Refining my current opening repertoire I seek sources for the following variations:

1.e4 d6 2.d4 Nf6 3.Nc3 c6
1.e4 d6 2.d4 Nf6 3.Nc3 e5 4.Nf3 Nbd7 5.g4!?
